Registration Information
As part of your registration process, you will be required to fill out Patient History Questionnaire .
Feel free to review this form so you can answer all the questions as accuratley as possible, or print it out - fill it out- and bring it in with you.
You will also be required to fill out a patient information sheet with all your pertinent data.
The single most important thing you can do to keep from getting sick and to stop the spread of disease-causing germs is to wash your hands often. By frequently washing your hands, you wash away germs that you may have picked up from other people or from contaminated surfaces.
